The Department of Mental Health team has developed a recorded training on how to  document Mental Health Prevention Services (MHPS).  The purpose of the video is to guide clinics through the documentation process for screening and services. Please remember that the documentation is required under the project and should include the following elements: 

• Date of Service
• Name of Rendering Provider 
• Duration of Session in Minutes 
• Procedure Code  
• Service Modality (Individual or Group)
• Face-to-face or Telephonic
• Patient Name and MHLA PID#
• Name of Prevention Practice (ex. Prolonged Engagement) 
• Name of Curriculum (Stress, TIC or Grief and Loss) 
• Note Narrative 
• Staff Signature
