Mid-Valley Comprehensive Health Center is charting bold new ground in diabetic eye care and tele-medicine to design more patient-centered services for our community.
Saving the Vision of Diabetic Patients
Diabetic disease is the leading cause of blindness among working-age Americans. Diabetes is the most prevalent chronic disease in our population. Improving eye care for diabetic patients improves their lives and is known to save money for the patient, our health system and society. At Mid-Valley CHC, we implemented new guidelines and procedures to meet increasing demand, which resulted in a 60% increase in clinical capacity. Increasing capacity at the Mid-Valley CHC Retinal Laser Clinic decreased clinic wait-time from more than one year to approximately four weeks. Patients with progressive eye disease are treated on-site with timely laser procedures preventing costly complications.
Using Tele-Medicine to Provide Access for Dermatology Services and Care
In a community needs assessment of access to specialty services, Dermatology was identified as one of the top five services in high demand, but was less accessible to patients. Together with the Valley Care Community Consortium, Mid-Valley CHC and various Community Partners worked to reduce the wait times and increase access to Dermatology through the use of a Teledermatology process. The team effort significantly decreased appointment wait times and increased patient access to this care.
