If you are interested in receiving services at Rancho Los Amigos National Rehabilitation Center, our Patient Access Center (PAC) team is here to help guide you through the process.

Outpatient Medical Services

The first step to receiving outpatient medical services at Rancho Los Amigos is through our referral office. Please work with your provider to complete the Outpatient Referral Services form, Patient Demographics form and fax cover sheet.

Outpatient Therapy Services

Interested in receiving Outpatient Therapy at Rancho Los Amigos? Please complete the Community Referral and Patient Information forms.

Inpatient Rehabilitation Services

People who are at home and would like to be evaluated for our inpatient rehabilitation program should start with an appointment in Rancho’s Physical Medicine and Rehabilitation clinic. To request an appointment, work with your provider to complete the Outpatient Referral Services form, Patient Demographics form and fax cover sheet.

The forms and the requested medical reports should be faxed back to us. Our team will guide the referral through all aspects of the process and make every effort to reach out to you to discuss your coverage benefits and any out of pocket costs.

Rancho Los Amigos accepts most insurances and we can negotiate single case agreements with health plans we do not have contracts with. We encourage you to contact your health plan with any questions you may have regarding your policy and to inquire if Rancho Los Amigos is an option for you.
