The LA Health Portal is a free and secure website and mobile app designed to connect you to your doctors and your health information. With the portal, you can manage appointments, speak to a provider, and review or renew your medications.

Download the new LA Health Portal App and log in using your existing patient portal username and password. Call or email us with questions!

Self-Enrollment is available for patients 18 and over.

In order to self-enroll, you will need:

  • Your existing medical record number, cell phone number, or email address
