Primary care services at Long Beach Comprehensive Health Center include adult, family and pediatrics. There is an Urgent Care available and also some specialty clinics, including OB-GYN/women’s health, HIV/Immunology, Dermatology, Podiatry, Anti-Coagulation, Ophthalmology, Optometry and Dental clinics, as well as an on-site pharmacy, radiology, lab and social work.
Services are provided through the Patient-Centered Medical Home (PCMH) model where the same medical home team members coordinate your care regardless of the setting. Your team members include:
Provider (MD, NP) – helps you maintain good health by listening to your health issues and providing evidence-based treatment and advice
Care Manager (RN) – provides high-risk patients with individualized care, including patient/family education and self-management support
Care Giver (RN)– Helps your medical home team care for you at the time of your clinic visit
Health Coordinator (CMA, LVN) – Assists the team during your clinic visit by taking your medical history and vital signs
Service Coordinator (RN) – Works with managed care plans and outside health agencies to help coordinate your care and ensure your needs are met
On the recommendation of your primary care provider, you may be referred to a specialist, such as a cardiologist, urologist or dermatologist, for treatment when you have a serious medical condition. For example, if you have a heart condition, you might be referred to a cardiologist. For a foot problem, you may be referred to a podiatrist. These specialists work with your primary care medical home provider to manage your care and make sure you get the treatment you need.
Your Medical Home team will help coordinate your care across all settings throughout the DHS network, as needed. The DHS Network includes hospitals, urgent care centers, emergency departments, behavioral health providers and specialty sites throughout Los Angeles County.
Youare the most important member of the health care team. It is important that you be active in your health care and work with your Medical Home team to develop a plan to maintain good health. At each of your visits, bring all of your current medications and let us know if your contact information has recently changed. If you were seen by another health provider, please bring any discharge papers you received or copies of your medical records.
