The Ambulatory Care Network (ACN) of Health Services is committed to providing world-class, compassionate health care that is safe and accessible for everyone – regardless or ability to pay.

Whether you need immediate care, a check-up, or ongoing care for a chronic medical condition, ACN offers a range of services through its integrated health system of 24 clinics across Los Angeles County. Our community-based health clinics offer quality primary care and specialty care services for the whole family right in your community. Services include urgent care, primary care, surgical, specialty clinics, behavioral health services – including mental health and substance use disorder services – diagnostic and ancillary services – including lab, radiology, and more. Our health centers also offer a variety of wellness and education programs that include classes on diabetes, asthma, nutrition, and much more.

Our network of exceptional primary care teams and specialists are dedicated to improving the health and wellness of patients and the communities we serve by delivering caring, compassionate, and quality health care to everyone.

Patient-Centered Medical Home
Through our patient-centered medical home model, your care is delivered by the same team of health care professionals who support your health and wellness needs. Your Medical Home team will help coordinate your care across all settings throughout the Health Services. The Health Services includes hospitals, urgent care centers, emergency departments, behavioral health providers, and specialty sites throughout Los Angeles County.

Juvenile Court Health Services (JCHS)
The ACN also oversees the Juvenile Court Health Services (JCHS), which provides quality health care to youth placed in any of the Probation Department’s juvenile detention halls and residential treatment camps. From placement to release, JCHS ensures that youth have access to complete health care services. Because the JCHS patient population ranges from youth with common healthcare concerns to those with high-risk issues, JCHS works closely with the Los Angeles County Probation Department and the Los Angeles County Department of Mental Health to address all healthcare needs. JCHS also continues to provide services to youth after they are released so that they can continue to thrive physically and mentally.

Recognition
Sixteen of the Ambulatory Care Network’s Patient Centered-Medical Homes are recognized by the National Committee for Quality Assurance (NCQA). NCQA is a private, nonprofit organization dedicated to improving health care quality. To be recognized by NCQA, clinics must demonstrate that they meet specific standards that show that we are functioning as medical homes. These standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement.
