Our Community Health Centers are committed to providing exceptional, compassionate healthcare that is safe and accessible for everyone regardless of ability to pay, insurance coverage, or immigration status.

Our health centers and clinics are part of LA Health Services, Los Angeles County’s integrated public healthcare system. The system includes four hospitals, more than 20 community health centers, Emergency Medical Services (EMS), Correctional Health Services, and community programs serving as the safety net for the 10 million residents of Los Angeles County.

Across more than 20 community health centers teams provide primary care, preventive services, specialty care, behavioral health, and chronic disease management. These sites connect patients to the broader LA Health Services system, ensuring seamless access to hospital care, specialty services, and community-based support when needed.

By bringing care closer to where people live and work, this network helps patients stay healthy, manage ongoing conditions, and navigate coordinated care across the system. As part of LA Health Services, it provides extraordinary care to all.

Patient-Centered Medical Home
Through our patient-centered medical home model, your care is delivered by the same team of health care professionals who support your health and wellness needs. Your Medical Home team will help coordinate your care across all settings throughout LA Health Services.

Changes to Medicaid and Medi-Cal funding have pushed LA Health Services into a budget crisis. To help protect patient care, make better use of our resources, and help keep costs down, we have relocated services of three of our community health centers.
