Patient Services
Primary care services at Antelope Valley Health Center (AVHC) include care for adults and children. Services are provided through the Patient-Centered Medical Home (PCMH) model where your medical home team coordinates your care regardless of the setting.
On the recommendation of your primary care provider, you may be referred to a specialist, such as a cardiologist, urologist or dermatologist, for treatment when you have a more complex medical condition. For example, if you have a complex heart condition, you might be referred to a cardiologist. Some specialists are located outside the Antelope Valley. These specialists work with your primary care provider to manage your care and make sure you get the treatment you need.
