To schedule an appointment with your provider, please contact us at one of the facility numbers listed below. You can also message your doctor or schedule an appointment through the LA Health Portal, login here.

If you do not have access to LA Health Portal and would like to enroll, please visit our self-enrollment page.

Do you need help getting to your medical appointments? As a Health Services patient, you have options to get to your doctor’s office. If you don’t have your own private car and public transportation doesn’t work for you, visit our Transportation page.
