Geriatric Medicine Services

What is Geriatric Medicine?: Geriatric medicine is a form of primary care specifically designed to meet the needs of older adults, generally aged 65 years and older. We emphasize maintaining function, independence, and quality of life. In addition to general primary care, geriatric care includes a comprehensive assessment and treatment for conditions that primarily affect older adults, such as: maintaining activities of daily life, memory loss, falls, urinary problems, vision and hearing loss, advance care planning, and medication safety. Geriatrics providers provide physical evaluations, medication monitoring, drug prescriptions, referrals to specialists, and oversee the total care of their patients. The Geriatrics care team is interdisciplinary, and engages the older adult patient, their family members and caregivers in their care.

Geriatrics care team members undergo specific training in the medical, psychosocial, and preventative team-based care specific to older adults. They strive to improve quality of life and wellness throughout the aging process. Geriatrics providers are known as Geriatricians and may be board certified in this specialty.

How can I get services?: If you are a current DHS patient aged 65 years old or older but have not been assigned to a Geriatrics provider, talk to your current medical provider to see if a Geriatrics clinic is the right place for your ongoing care needs. If you are not currently a DHS patient and do not have a primary care provider, contact or visit one of the DHS sites listed below that offer Geriatric Medicine to find a provider and get linked to services. 

Please note that financial screening is required at each of these locations prior to seeing a provider or making an appointment. Eligibility staff will help you determine all available coverage and payment options, including health insurance (MediCal and Medicare) options.
