It’s best for the requestor to know the location where the patient received care from, so that the request can be submitted to the correct facility. Patients or their personal representatives can request to access health information by either submitting a request in person, by mail, or via fax to the site where they received medical care.

When submitting a request, it is also important to submit the correct form when placing a request for medical records. Below you will find some information on our most commonly used forms with a brief description of the forms purpose.

ACCESS TO PROTECTED HEALTH INFORMATION

This form can be used when a patient or their personal representative is requesting to receive copies or access the legal medical record from the site where the patient received care. To print this form, please click here.

AUTHORIZATION TO USE AND DISCLOSE PROTECTED HEALTH INFORMATION

This form can be used when a patient or their personal representative wants to give permission for their protected health information to be released to another person or location. To print this form, please click here.

If the request is sent by mail, we will need to verify the applicants identity and authority to request the information, so we recommend sending in a copy of a government issue identification card with the request. Note: you don’t have to reveal the identity card numbers when submitting your request.

As per applicable law, there may be a fee for obtaining medical records. Fees are:

  • Up to $0.25 per page copied on paper
  • Up to $6.00 for records placed on compact disc
  • Up to $0.50 per page for records that are copied from microfilm
  • $15.00 flat fee if the records are delivered to an attorney or the attorney’s representative for inspection or photocopying at the record custodian’s place of business
  • Copy fees may be waived if the request is for relevant portions of a patient’s record to support an appeal regarding eligibility for a public benefit program. A written request and proof that the records are needed to support the appeal are required to waive the fee.

An invoice may be sent to the requestor if the request requires payment.

Medical Record Request Processing Timeframes

Patient or patient representative request for medical record are usually processed within 15 days after we receive a complete and accurate written request.

In the event where a request for medical records is incomplete or incorrectly completed, the request may be rejected. We will try our best to notify the requestor of the rejection in a timely manner.

As a covered entity under the Health Insurance Accountability and Portability Act, the Health Services understands that the sharing protected health information with other covered entities helps builds professional relationships, assists with treatment, payment, and health care operation functions. Our goal is to provide request for protected health information to other health care providers in a timely manner, and to help us with this goal, we have provide you with some useful tips that we hope will assist you with submitting your request.

The Health Services operates various facilities across Los Angeles County and we often receive medical record request with the wrong facility name or with names of providers who don’t work at the facility where the request was submitted to. Please ensure that your request is accurate and complete and is sent to the correct Health Services facility. To view our office locations, please click here.

In order for us to process your request, please ensure that your request has sufficient information to help us identify the patient and clearly states what type of protected health information is needed. We highly recommend that you also provide us with the reason for your request,  Example: Requesting PHI for continuing of care purposes.

In the event where your request is needed STAT, please contact the facilities Health Information Management Unit and inform them of the need. Health Information Management staff are trained to handle such request but will first need to verify the requestors identity and authority to adhere to privacy laws.

Depending on the nature and content of the request, we usually process patient or patient’s representative disclosure request within 15 days after receiving a complete and accurate written request.

Olive View-UCLA Medical Center volunteers play a key role in our caring healthcare team.  Volunteers provide thousands of service hours each year to Olive View hospital and help support our patient care mission.  Consider the many ways you can make a difference through volunteering.

Volunteering provides an exceptional opportunity to take an active role in your community, build skills, make a difference, and explore career opportunities.  Countless volunteers have gone on to pursue careers in the healthcare field as a result of their volunteer experience.  Most volunteer shifts are flexible to your schedule.  You will be eligible to receive a letter of recommendation and documentation of community service after the completion of 50 hours of service.

Make a Difference – Helping others is one of the most rewarding experiences.

Gain Experience – Volunteering exposes volunteers to multiple healthcare careers.  Colleges and Universities often consider an applicant’s experience as a volunteer.

Meal Ticket – Is provided after four hours of service daily.

Medical Center functions – Volunteers may attend various events, including the Annual Volunteer Banquet, holiday parties, special fairs and events.
Discount Tickets – Volunteers may purchase discount tickets through the March of Dimes coordinator to theme parks, Sporting Events, Movie Theatres and other area attractions.

Educational programs/Health Screenings – Many health and wellness programs are offered at no cost to volunteers.

There are a few steps you must take to become a volunteer:
1.  Apply on-line
2.  You will be invited to attend an Interview with the Volunteer Director
3.  Make appointment for Live Scan with Volunteer Office
4.  When Live Scan clears make appointment for T.B testing
5.  When volunteer office receives your I.D. Badge you will be invited for a 90 minute orientation
6.  Meet with the Volunteer Director to be placed in an area of your interest.

Please be advised that while we are currently accepting applications, it can take anywhere from 2-4 weeks to complete the application process before you may actually begin volunteering.

Adult Volunteer:

Must be 18 years of age or older
A sincere interest in volunteering
Must complete a minimum of 100 hours in a 6 month time frame, with a 4 hour commitment per week
Must be able to clear background check (Live Scan)
Must submit Immunization Records to our Employee Health and clear T.B. testing
Attend a mandatory 90 minute Orientation
Must be able to complete all clearances and begin volunteering within 30 days of applying
Must be willing to commit to the schedule that is created by yourself and the Director of Volunteers.

Junior Volunteer

Must be 14 years of age, but not older than 17
A sincere interest in volunteering in a hospital setting
Must be currently enrolled in school
Must complete a minimum of 100 hours in a 6 month time frame, with a 4 hour commitment per week
Application must be signed by a parent or guardian
