Rancho<\/span>\u00a0<\/span><\/i>KnowBarriers\u00a0<\/span><\/i>is one of the largest peer mentoring rehabilitation programs of its kind.\u00a0<\/span>\u00a0<\/span>This unique program blends the life experience of former Rancho patients, who have achieved life success following their injury and\u00a0<\/span>received\u00a0<\/span>specific training in the skills of peer mentoring and life coaching.<\/span>\u00a0<\/span><\/p>\n

Peer Mentoring Services<\/span><\/b>\u00a0<\/span><\/p>\n

KnowBarriers<\/span><\/i>\u00a0<\/span>Peer Mentors play an integral role in the rehabilitation and medical services at Rancho.\u00a0<\/span>They\u00a0<\/span>work alongside the clinical professionals to provide patient education, skill demonstration, resource referral, emotional support and\u00a0<\/span>display\u00a0<\/span>authentic life\u00a0<\/span>models of living with a disability<\/span>.<\/span>\u00a0P<\/span>erhaps most importantly<\/span>,\u00a0<\/span>the\u00a0<\/span>team of\u00a0<\/span>P<\/span>eer\u00a0<\/span>M<\/span>entors motivate our patients to believe in their own abilit<\/span>ies\u00a0<\/span>to achieve a<\/span>\u00a0positive quality\u00a0<\/span>of life, despite their situation.<\/span>\u00a0<\/span><\/p>\n

\u00a0<\/span>Support Groups<\/span><\/i><\/b>\u00a0<\/span><\/p>\n

KnowBarriers<\/span><\/i>\u00a0Peer Mentors co-facilitate the majority of Rancho support groups.\u00a0 Peer\u00a0<\/span>Mentors\u00a0<\/span>develop a rapport with patients during their rehabilitation program and encourage the patients to continue their recovery by attending the support\u00a0<\/span>groups.<\/span>\u00a0<\/span>\u00a0Patients often feel more comfortable attending knowin<\/span>g there will be a familiar face to welcome them.<\/span><\/p>\n

Resource Center<\/span><\/i><\/b>\u00a0<\/span><\/p>\n

The Rancho Resource Center serves patients, visitors and staff.\u00a0 Services include computers with internet access,\u00a0<\/span>information on Rancho and community-based programs to support people with disabilities, information on current events, <\/span>and individual appointments with a Peer Mentor<\/span>.<\/span>\u00a0<\/span>KnowBarriers<\/span><\/i>\u00a0Peer Mentors<\/span>\u00a0provide individual<\/span>ized<\/span>\u00a0support to\u00a0<\/span>assist patients and family members to\u00a0<\/span>research resources, apply for services, and to enroll and navigate the LA Health Patient Portal. <\/span>\u00a0<\/span><\/p>\n

The Resource Center is open Monday \u2013 Thursday,<\/span> 9<\/span>am \u2013 3<\/span>pm.\u00a0<\/span>\u00a0<\/span><\/p>\n

Pain Coping Skills\u00a0<\/span><\/i><\/b>\u00a0<\/span><\/p>\n

People with disabilities may be susceptible<\/span>\u00a0to chronic pain<\/span>\u00a0due to the\u00a0<\/span>physiologic\u00a0<\/span>nature of their injury\/illness or the\u00a0<\/span>additional physical strain of using our body in a way other than how it was intended.\u00a0<\/span>\u00a0<\/span><\/p>\n

The Pain Coping Skills classes provide patients with an opportunity to learn more about the physiology of chronic pain<\/span>, as well as<\/span>\u00a0<\/span>a wide array of\u00a0<\/span>strategies to address it from\u00a0<\/span>a person\u00a0<\/span>with a disability who has learned to manage their own chronic pain. The\u00a0<\/span>KnowBarriers<\/span><\/i>\u00a0<\/span>Peer Mentor\u00a0<\/span>provides information developed by clinical experts in pain management. These classes are part of a continuum of pain management services offered at Rancho.<\/span>\u00a0<\/span><\/p>\n

Life Coaching Services<\/span><\/b>\u00a0\u00a0<\/span><\/b>\u00a0<\/span><\/p>\n

KnowBarriers<\/span><\/i>\u00a0<\/span>Life Coaches\u00a0<\/span>are individuals with disabilities who have specialized training to assist patients to develop tools to set and achieve their life goals.\u00a0<\/span>\u00a0<\/span><\/p>\n

Achieve a Goal course<\/span><\/i><\/b>\u00a0<\/span><\/p>\n

Through a series of classes, participants will work in small group to make progress on their individual goals. The\u00a0<\/span>L<\/span>ife\u00a0<\/span>C<\/span>oach will introduce a new skill each week for the participants to apply toward<\/span>s<\/span> achieving their goal<\/span>s<\/span>.\u00a0<\/span><\/p>\n

Individualized Life Coaching<\/span><\/i><\/b>\u00a0<\/span><\/p>\n

Life <\/span>Coaches\u00a0<\/span>provide goal setting, problem solving, peer support, accountability and resource referral.\u00a0<\/span>Services are provided in person and virtually on a weekly basis. Participants<\/span>\u00a0commit to attending life coaching sessions and working toward their goals between sessions.\u00a0<\/span>Graduates of<\/span>\u00a0the program have demonstrated improved confidence, decreased level of depression and\u00a0<\/span>an\u00a0<\/span>overall improved quality of lif<\/span>e.\u00a0<\/span>\u00a0<\/span><\/p>\n
