Primary Care Medical Homes

Rancho Los Amigos National Rehabilitation Center provides Primary Care services through Patient-Centered Medical Homes.

The Patient-Centered Medical Home, or PCMH, is a team-based approach to providing Primary Care, involving a highly collaborative group of nurses, doctors, and other specialists who work together to meet the needs of their patients. Drawing upon our rich history and our expertise in Rehabilitation Medicine, several Patient-Centered Medical Homes are designed to cater specifically to persons with unique care needs.

Specialty Clinics

Rancho Los Amigos National Rehabilitation Center offers many Specialty Services all in one convenient location.

Each of our medical home clinics is connected to a wide array of Specialty Clinics, from Dental Services to Diabetes Care, Orthopedics to Ophthalmology. There is rarely a need to travel to a different facility or location for the Specialty Service that meets your unique needs. Your primary care provider can determine if you would benefit from consultation with one of our specialists and make a referral if needed.

Rehabilitation Therapies

At Rancho Los Amigos, Rehabilitation does not end when a patient returns home.

Outpatient Occupational Therapy, Physical Therapy and Speech Therapy offer a full range of traditional Outpatient Therapy Services via in-person and telehealth visits to meet a variety of patient needs.  Specialty programs include Driver Rehabilitation and Training, Work Rehabilitation, Lifestyle Redesign, Seating Center, the Center for Applied Rehabilitation Technology (CART), Balance and Vestibular, Musculoskeletal and Orthopedic Physical Therapy, and Pelvic Floor Dysfunction/Incontinence. These specialty services offer additional expertise to assist individuals in successfully transitioning home and reintegrating into the community. Health Services (HS) patients as well as community patients are welcome. A physician’s referral to Occupational Therapy, Physical Therapy, or Speech Therapy is required. HS patients may obtain a referral by requesting one from their primary care provider. Once the referral is received you will be contacted to schedule an appointment. Community patients may contact the Outpatient Referral Office at (562) 385-6536 to begin the referral process.
