Quality Improvement & Patient Safety Related Concerns 

QIPS works with the Health Service hospitals and comprehensive clinics to respond to patient safety concerns.   

If the facility in question is within the Health Services jurisdiction you may report a patient safety concern to QIPS by sending an email to patientsafety@dhs.lacounty.gov.   You should hear from us within 3-5 business days. 

If your needs and/or concerns are not related to patient safety, please contact the specific involved facility or clinic directly.  Please click here to obtain DHS facilities’ contact information.

Joint Commission Office of Quality Monitoring 

The public may contact the Joint Commission’s Office of Quality Monitoring to report any concern or make a complaint about a health care facility that is accredited by the Joint Commission by calling (800) 994-6610 or emailing: complaint@jointcommission.org. 

California State Medical Board 

Medical Doctors are licensed and regulated by the Medical Board of California. Specific concerns regarding Medical Doctors may be reported to the Medical Board of California.
