QIPS works with the Health Service hospitals and comprehensive clinics to respond to patient safety concerns.
If the facility in question is within the Health Services jurisdiction you may report a patient safety concern to QIPS using our hotline phone number (213) 288-7233 or email address firstname.lastname@example.org. You should hear from us within 3 business days.

Medical Doctors are licensed and regulated by the Medical Board of California. Specific concerns regarding Medical Doctors may be reported to the Medical Board of California.
