Please note that the following facilities have consolidated their Release of Information locations, and require subpoenas to be delivered to their main location.<\/p>\n

  1. Glendale & San Fernando Health Centers subpoenas are to be delivered to Mid Valley Comprehensive Health Center.<\/li>\n
  2. Torrance & Wilmington Health Center subpoenas are to be delivered to Long Beach Comprehensive Health Center.<\/li>\n<\/ol>\n

For a listing of our facilities contact numbers, please click\u00a0here<\/a>.[\/vc_column_text][\/vc_tta_section][vc_tta_section title=”QUESTION 3: WHAT IS YOUR PROCESSING TIMEFRAMES FOR PATIENT INITIATED REQUEST?” tab_id=”1604162707725-53d78668-3e3d”][vc_column_text]Depending on the nature and content of the request, we usually process patient or patient\u2019s representative disclosure request within 15 days after receiving a complete and accurate written request.[\/vc_column_text][\/vc_tta_section][vc_tta_section title=”QUESTION 4: WHAT ARE YOUR FEES FOR ATTORNEY REQUEST FOR MEDICAL RECORD?” tab_id=”1604162706894-52474583-a4ba”][vc_column_text]We charge a $15.00 flat fee for attorney request for medical records.[\/vc_column_text][\/vc_tta_section][vc_tta_section title=”QUESTION 5: HOW CAN MY CLIENT OBTAIN A FREE COPY OF THEIR MEDICAL RECORD?” tab_id=”1604162706232-3e87ad10-57af”][vc_column_text]Copy fees may be waived if the request is for relevant portions of a patient\u2019s record to support an appeal regarding eligibility for a public benefit program. A written request and proof that the records are needed to support the appeal are required to waive the fee.<\/p>\n
