Community Partners can access the addresses and phone numbers of all their active and inactive participants by downloading the Medical Home Summary on One-e-App, which makes it easy to send renewal reminders or encourage inactive participants to re-apply. Only users with the System Administrator profile have this capability. Just follow these directions.

1. After logging in, select the System Administrator User Type.
2. On the Welcome page, click on the Medical Home Summary link on the left panel.
3. Once on the Medical Home Summary page, select Approved for the Disposition Status to get active members. Select Denied and Disenrolled for inactive members.
4. Leave everything else blank and click on View Summary.
5. When the number of records is returned, click Save to Excel to download the list.

The ins_end_date section (Column I) on the list will show upcoming renewal due dates for active participants.
