Patient Services
Primary care services at Lake Los Angeles Community Clinic (LLACC) include care for adults and children. Services are provided through the Patient-Centered Medical Home (PCMH) model where the your medical home team coordinates your care regardless of the setting.
On the recommendation of your primary care provider, you may be referred to a specialist, such as a cardiologist, urologist or dermatologist, for treatment when you have a more complex medical condition. For example, if you have a complex heart condition, you might be referred to a cardiologist. Some specialists are located outside the Antelope Valley. These specialists work with your primary care medical home provider to manage your care and make sure you get the treatment you need.
