We hold monthly bi-lingual meetings with leadership partners and frontline staff to provide input from the patient and community perspective on healthcare improvement projects. Here are a few of our projects.[\/vc_column_text][\/vc_column][\/vc_row][vc_row][vc_column][vc_column_text]<\/p>\n<ul>\n<li>Outpatient Pharmacy services improvement project<\/li>\n<li>Clinical Tower Pharmacy waiting room re-design<\/li>\n<li>Pre-surgical process re-design<\/li>\n<li>New admission patient education video<\/li>\n<li>Primary care drive-through vaccine project<\/li>\n<li>\u201cBetter Normal Nancy\u201d telehealth visit improvement \u2013 names after our very own Patient Family Advisor, Nancy Velasquez<\/li>\n<li>Personal Protective Equipment (PPE) ICU Project<\/li>\n<li>Language Sensitive Care Project<\/li>\n<li>Community Outreach Strategies<\/li>\n<li>SMS Appointment Readiness Text Messaging<\/li>\n<li>GI Lab Procedural Appointment Process Improvement<\/li>\n<li>Opioid Stewardship<\/li>\n<li>EDx<\/li>\n<li>Language Access and Inclusion Signage<\/li>\n<li>DHS PFAC Telehealth Task Force<\/li>\n<\/ul>\n<p>[\/vc_column_text][\/vc_column][\/vc_row][vc_row][vc_column][vc_column_text]Patient and Family Advisor interviews are held monthly. Applications for referrals\/candidates are available via hard copy throughout the Medical Center and electronically on the left car in English, Spanish, and Chinese. English and Spanish forms can also be accessed with their respective QR codes found at the bottom of this page.<\/p>\n<p>As of 2021 our Patient Family Advisors has volunteered 178 hours, provided the patient perspective and membership on our ongoing hospital committees and participated in 22 QI projects![\/vc_column_text][vc_empty_space][vc_separator][vc_empty_space height=&#8221;15px&#8221;][\/vc_column][\/vc_row][vc_row][vc_column width=&#8221;1\/3&#8243;][vc_single_image image=&#8221;10116&#8243;
