CBEST provides targeted advocacy to assist individuals who are homeless or at risk of homelessness in obtaining sustainable income through programs such as Social Security Income (SSI) or Social Security Disability Insurance (SSDI).

 

Services Provided: 

Eligible individuals receive comprehensive-benefits advocacy services. The program, known as Countywide Benefits Entitlement Services Team (C-BEST), includes outreach to participants who are homeless and who are potentially eligible for SSI/SSDI benefits, such as:

  • Screening and assessment to ensure participants meet medical and non-medical requirements for SSI/SSDI
  • Coordination of health and mental health records to support the SSI/SSDI application
  • Assistance with obtaining additional health and mental health documentation as needed
  • Legal consultation for complex claims, appeals, and hearings
  • Internal quality assurance reviews
  • Liaising with Social Security Administration and State of California Department of Social Services Disability Determination Services
  • Filing for reconsideration of denied applications
  • Coordination with health, behavioral health and Permanent Supportive Housing – Intensive Case Management Service providers who serve the participant

 

Eligibility Requirements:

  • Over 65 years of age
  • Have a physical or mental health disability
  • Not currently receiving SSI or SSDI benefits

 

Length of Service:

The CBEST program has no average length of service and is dependent on the need of the beneficiary.
