This includes County employees \u2013 interns, residents and fellows; and trainees rotating from outside \u2013 visiting medical students and visiting residents.<\/p>\n<p>Required BLS Certification:<\/p>\n<p>Per hospital policy, all residents and fellows will be required to present a valid BLS certification card in order to obtain their Harbor ID badge. BLS certification must be registered with the\u00a0<a href=\"https:\/\/atlas.heart.org\/home\">American Heart Association (AHA).<\/a><\/p>\n<h3>Information and Resources for Accepted Residents &amp; Fellows<\/h3>\n<h4><strong>Benefit \u2013 Choices<\/strong><\/h4>\n<p>County-paid Harbor trainees participate in the Choices benefits plan. As a Choices participant, you get a monthly benefits allowance. The amount you receive is based on the number of dependents you choose to enroll for coverage. You use this monthly allowance to go \u201cshopping\u201d for the benefits you need. If you have any allowance left over after you go benefits shopping, this leftover allowance will be added to your paycheck as taxable pay.<\/p>\n<p>You can log in using your six-digit ID and the two-digit month and four-digit year of your birth as your PIN (for example, if your birth month\/year is October 1964, your PIN is 101964). You will then be asked to create an eight-digit password.<\/p>\n<p><u>Benefits \u2013 Effective Dates<\/u><\/p>\n<p>You will be required to extend your present health care coverage (i.e. COBRA). For more information on what a trainee qualifies for, you must contact the FMLA unit directly.<\/p>\n<h4><strong>California DMV<\/strong><\/h4>\n<p>Make an appointment to get your drivers license and register your car otherwise you will be waiting for hours. Visit www.dmv.ca.gov to make an appointment or call 1-800-777-0133. Getting a drivers license will require an eye exam and a written exam.<\/p>\n<h4><strong>Banks<\/strong><\/h4>\n<p>Banks usually open accounts with a picture ID and major credit card but you should have cash and\/or travelers checks to use until your account is opened and deposited funds have cleared.<\/p>\n<p><em><a href=\"https:\/\/www.southlandcu.org\/\">Southland Credit Union<\/a><\/em>\u00a0will open accounts with a picture ID and a copy of your Appointment Agreement\/Contract Credit. Detailed instructions must be followed to activate an authorized contractor profile online with a \u201cLos Angeles County Department of Health Services Information Sheet\u201d for every rotation. Once the online information sheet is complete, the rotation Program Coordinator will review upon submission. After HR review and approval, the rotation Coordinator will provide instructions for the HR Live scan\/fingerprinting process.<\/p>\n<p><strong>1- Harbor-UCLA Human Resources Registration Process:<\/strong><\/p>\n<p>Per the\u00a0<a href=\"https:\/\/file.lacounty.gov\/SDSInter\/dhs\/1200483_Policy-201-Hospital-Staff-Identification-Badge-Rev-6-21.pdf\" target=\"_blank\" rel=\"noopener\">Harbor-UCLA hospital policy<\/a>,\u00a0all personnel must wear a valid Harbor-UCLA Medical Center hospital badge for identification purposes. After GME office registration is complete, the visiting resident will receive an email confirmation to visit onsite Human Resources to retrieve a badge on the first day of service. The ID badge enables the trainee to;<\/p>\n<ol>\n<li>receive meals at no charge in the Doctor\u2019s Dining Room<\/li>\n<li>bypass security at all hospital entrances<\/li>\n<li>gain access through physician-only areas via elevator\/door activation<\/li>\n<li>acquire assistance at the site\u2019s Information Systems office<\/li>\n<li>check out Harbor-UCLA scrubs<\/li>\n<\/ol>\n<p>Required BLS Certification:<\/p>\n<p>Per hospital policy, all visiting residents will be required to present a valid BLS certification card in order to obtain their Harbor ID badge. This should be followed up with a visit to employee health to ensure you have cleared.<\/p>\n<p>For Visiting Residents\/Fellows who are Non-County workforce members (with an c number), please submit a\u00a0<a href=\"https:\/\/file.lacounty.gov\/SDSInter\/dhs\/1200495_E2-Pre-PlacementTBHistoryEvidenceofImmunityPacket.pdf\">Non-County Employee Health Clearance form<\/a>\u00a0to the Employee Health office to obtain the required health clearance. Therefore, we are requesting that all medical schools and affiliate institutions begin sending COVID-19 vaccination status for all your rotators currently on our campuses and making sure each Workforce member\u2019s COVID-19 vaccination status is included in the onboarding requirements prior to beginning their rotation in the near future.<\/p>\n<p>How to Submit Proof of Vaccination:<br \/>\nEmployee Health Services is collecting proof of vaccination for all employees and contractors who received vaccine outside of DHS. This registration facilitates Non-County employee registration process and other site-specific amenities applicable to visiting residents and fellows.<\/p>\n<p><strong><em><u>Effective July 1, 2026<\/u><\/em><\/strong> <em>\u2013 All GME packets must be submitted at least <\/em><strong><em><u>6 weeks prior to the start of a rotation<\/u><\/em><\/strong> <em>to ensure paperwork is processed in time for the start of the rotation.<\/em><\/p>\n<p><strong><em>**Failure to submit GME packets on time will result in the rotator not being cleared and the rotation required to be rescheduled.**<\/em><\/strong><\/p>\n<p>The following documentation must be emailed to the site coordinator for forwarding to the GME office;<\/p>\n<ol>\n<li><a href=\"https:\/\/file.lacounty.gov\/SDSInter\/dhs\/1206232_AffiliatingResident.FellowQuestionnaireFeb.26FILLABLE.pdf\">Affiliating Resident\/Fellow Questionnaire REV. 2\/2026<\/a><\/li>\n<li>Proof of current California Medical License*\u00a0<em>(photographic images will not be accepted).<\/em><\/li>\n<li>Proof of current DEA Certificate (if applicable),\u00a0<em>(photographic images will not be accepted).<\/em><\/li>\n<li>Copy of Valid ECFMG Certificate (for International Medical Graduates),\u00a0<em>(photographic images will not be accepted).<\/em><\/li>\n<li>Copy of Medical School Diploma.<\/li>\n<li>Copy of Current CV (must include your training history).<\/li>\n<\/ol>\n<p><strong>ORCHID Training:<\/strong><\/p>\n<p>Visiting Residents will need to watch the YouTube ORCHID Training videos (2 videos) and complete the ORCHID Provider Test (1 test).<\/p>\n<p>The YouTube ORCHID Training videos and ORCHID Provider Test are accessible via links located in the following PDF:\u00a0<em><a href=\"https:\/\/file.lacounty.gov\/SDSInter\/dhs\/1200483_Policy-201-Hospital-Staff-Identification-Badge-Rev-6-21.pdf\" target=\"_blank\" rel=\"noopener\">ORCHID Training Videos &amp; Tests<\/a><\/em><\/p>\n<p>Please note, for those who are rotating in the Emergency Department Service (including Psychiatry) and Urgent Care, there are additional training videos and a test for you to complete (all are listed in the PDF above).<\/p>\n<p>Once you have completed the required training, please make sure to email a copy of the test and email it to your Department\/Rotation Coordinator.<\/p>\n<p><strong>Doctor\u2019s Dining Room (DDR) Registration:<\/strong><\/p>\n<p>Visiting Residents &amp; Fellows are eligible for meals, provided for the duration of the clerkship in the (DDR). Meals are accessed by using your Harbor ID badge. Please allow 24-48 hours from receipt of your Harbor ID badge for meal access.<\/p>\n<p><em>Unprofessional behavior will not be tolerated in the DDR.<\/em><\/p>\n<p><strong>Harbor-UCLA-Issued Scrubs:<\/strong><\/p>\n<p>Please report to the Linen Department located on the Basement floor, Room B303 of the main hospital to check out scrubs.<\/p>\n<p><em><strong>**You will be required to return your Harbor-issued scrubs directly to the Linen department at the end of your scheduled rotation.<\/strong><\/em><\/p>\n<p>______________________________________________________________________<\/p>\n<p>* California state law requires that all Medical School graduates obtain a training license within 180 days of program enrollment. Postgraduates having completed 36 months of training must obtain full licensure within 180 days of advanced program training enrollment or by the expiration of their PTL (whichever comes first). Postgraduates wishing to rotate at Harbor from outside the state, and who do not have a CA license, may be eligible to do so if they meet the Medical Board of California requirements.[\/vc_column_text][\/vc_tta_section][vc_tta_section title=&#8221;Medical Students&#8221; tab_id=&#8221;1747436308626-ec813d8b-a088&#8243;][vc_column_text css=&#8221;&#8221;]<\/p>\n<h2><strong>Medical Student Application Process<\/strong><\/h2>\n<p>Thank you for your interest in pursuing an elective at Harbor-UCLA Medical Center. You can download application materials and instructions from the\u00a0<em><a href=\"https:\/\/medschool.ucla.edu\/education\/md-education\/visiting-students\">UCLA Visiting Student<\/a><\/em>\u00a0webpage<\/p>\n<p>For full consideration, applications must be received in the course department at least 60 days prior to the start of the requested elective and must be properly completed. All incomplete applications will be returned.<\/p>\n<p>Upon Live Scan clearance and Employee Health clearance, all medical\u00a0 students must register with Harbor Human Resources office (Building 3-South) on the first day of their assigned clerkship to obtain a Harbor ID badge. Medical Students must register\/orient with their respective program\/department. Students must provide HR with a copy of their most current BLS certification. Students that have been live scanned at another LA County site for a previous clerkship rotation may not have to be live scanned again.
