Currently, I serve as Director of Intake Psychiatric Services at Correctional Health Services (CHS), where my teams provide services similar to Psychiatric Emergency Services by evaluating, treating, and triaging patients at the door of the LA County jails. I have been interested in Street Medicine and began working with homeless shelters and volunteer clinics early in my education and training. Over the years, I\u2019ve come to learn that our patients on the streets, in psychiatric hospitals, and in jails are often the same individuals cycling through the same revolving door, a reflection of systemic gaps and the urgent need for integrated healthcare. CHS cares for patients in the largest jail system in the world, with approximately 15,000 individuals in custody on any given day. LA County jails also function as the largest psychiatric institution in the U.S., housing approximately 6,500 individuals with psychiatric illness. For me, there\u2019s no better place to apply my training, knowledge, and skills than here, working with the sickest, most marginalized, and most vulnerable patient populations. My day-to-day work includes a mix of clinical and administrative responsibilities, as well as teaching the next generation of physicians rotating from various medical schools and postgraduate programs. [\/vc_column_text][vc_column_text css=””]Richard A. Murphy, MD, MPH is an Infectious Disease (ID) specialist with a particular interest in HIV and its complications. Dr. Murphy is also committed to patient-oriented research aimed at improving outcomes for people living with HIV: \u201cI currently serve as Physician Lead for HIV Programs in the LA County jails, where the typical census of people living with HIV is nearly 300 individuals, many of whom have recently been out of care and off treatment. My first position after fellowship was as an ID advisor for Doctors Without Borders, where I focused on antimicrobial resistance and malaria in precarious environments. I was ultimately drawn to Correctional Health Services (CHS), which I found to be uniquely positioned as a provider of last resort for extremely vulnerable patients in Los Angeles. Most days, I care for people with HIV and other infectious diseases in the medical clinics inside the jails. Primary care is also a substantial component of my practice. In addition, I maintain a clinical role at LA General Medical Center as part of the Infectious Diseases Division. One surprising and deeply rewarding aspect of working in CHS is the opportunity to care for a relatively young patient population. While much of internal medicine in the community focuses on older adults, CHS offers direct engagement with the needs of medically underserved people in their 20’s and 30’s, including those with substance use disorders, infectious diseases, STIs, psychiatric illness, and those seeking gender-affirming care. I also value the many opportunities for quality improvement and patient-oriented research within this setting. A key challenge of providing care in CHS is that medical decision-making can require negotiation with other systems and priorities. For this reason, physicians in CHS must be strong and consistent advocates for their patients. In the jail setting, it is not enough to hope that things will go well; it is essential to follow up and ensure that they do. Through this advocacy, physicians gain a deeper understanding of how the system functions, and when problems are identified, there is a genuine willingness at all levels to improve CHS and to use data-driven approaches to do so. For physicians seeking a role in medicine that combines service in one of the most critical components of the medical safety net, collaboration with mission-driven colleagues, ongoing skill development, and a work environment that is rarely dull, I strongly recommend applying for a position at CHS.\u201d[\/vc_column_text][\/vc_tta_section][vc_tta_section title=”Dr. Wendy Ruggeri” tab_id=”1746553067130-5e5d0e87-b1d0″][vc_column_text css=””]<\/p>\n

I am currently the Senior Physician overseeing Urgent Care\u00a0in Twin Towers, as well as<\/span> Primary Care Clinics at Pitchess Detention Center (PDC). I was led to Correctional Health Services (CHS) by a colleague who enjoys working here. I started as a Relief Physician, working in the Urgent Care a few times per month. With time I realized my experience as an ER doctor has proven to be extremely beneficial in treating the incarcerated population. As a Senior Physician I oversee a group of healthcare professionals who attend to urgent needs of our patients and, when possible, avoiding unnecessary transfers to tertiary hospitals. I do clinical work as well as administrative work to improve workflows and optimize care. Being a CHS physician is rewarding and poses unique challenges to practicing medicine. We may not have the same resources and availability for testing as outside clinics and we must work within the safety parameters issued by our custody partners. Upon joining the CHS team, physicians will find that you can make a difference with our patient population.
