Exclusive Provider Organizations (EPO) – Managed Care

Your EPO does not cover any services members get from out-of-network doctors and hospitals.  (Except for emergencies).

How much does it cost?

  • Monthly premiums
  • Co-payment and other out-of-pocket cost may apply
  • Annual deductible must be paid before your EPO starts covering your medical bills
  • You may be required to cover a percentage of the total charges for your medical bills

What is required?

  • You will be encouraged to use the insurance company's EPO network of preferred doctors
  • Only In-network healthcare services will be covered

What is covered?

  • EPO plans offer a wide range of healthcare services through a network of doctors who agree to provide services to members
  • With an EPO you will have coverage for a full range of preventive healthcare services than you would through another type of plan

Where do I go for care?

Before you can see a specialist at LA County DHS, you will need to obtain an authorization from your PCP or check with your EPO member services
