Monthly Demographic Reports

The Demographic Report summarizes the key demographic and enrollment data including age, ethnicity, language spoken and gender. Additionally, the total enrollment numbers are provided by Community Partner Medical Home. These reports will be maintained for the most recent twelve months of the program.
